Admissions
Through the Pennsylvania Person/Family Directed Support Waiver, we strive to aid individuals with an intellectual disability, autism, or developmental disability with living more independently in their homes and communities and to provide services that promote community living, including self-directed service models and traditional, agency-based service models.
Eligibility Criteria:
18 years or older
Diagnosis of an intellectual disability or developmental disability
Determined eligible for Medical Assistance (MA) in Pennsylvania
Meet the financial requirements as determined by your local County Assistance Office
We review all applicants on a case by case basis to determine if we can provide the supports and services required. One of our team members will respond to an applicant’s initial call or email.
